This taxon is known only from a single female specimen collected from the Akar (= Akah) River, Sarawak, Malaysia. As the specific collection locality on the Akah River could not be located, a distribution map cannot be completed for this species.

It is known only from the holotype, so there is no information on its population status.
The threats to this species are not known, but might include deforestation of lowland forest.
It is not known if it occurs in any protected areas.
This species is known only from a single specimen that is very similar to Pelophryne guentheri. The taxonomic status of P. macrotis requires additional specimens in order to confirm its validity (Inger 1966).
